Provides a variety of culturally-sensitive services that promote individual growth, healthy living, and disease prevention. Our mission is to improve individual, family, and community wellness by promoting achievable lifestyle changes that meet people where they are at.

We have a team of Certified Diabetes Educators and Registered Dietitians that work alongside providers, seeing patients point-of-care as well as during scheduled one-on-one visits.

Nutrition counseling is provided for the management or prevention of heart disease, diabetes, high blood pressure, high cholesterol, obesity, & other nutrition-related conditions. Some of our specialty services include:

Diabetes Management- consults provided one on one by our Certified Diabetes Educators, and facilitated in a group setting using DEEP-Diabetes Self-Empowerment Education.

Pregnancy counseling throughout pregnancy and the post-partum period for members enrolled in NAHC’s Perinatal Services.

Pediatric Weight Management-consults provided one-on-one, and through our Healthy, Active Families group education program for kids and their families. This group is conducted by both a Registered Dietitian and Family Nurse Practitioner.

Free Wellness Classes are provided to NAHC members at the community, including cooking and exercise classes. CLICK HERE for our current Class Schedule.

Community Collaborations

EYEPACS: Through a partnership with UC Berkeley, we have a Retinal Camera which allows us to screen patients for retinopathy, a major cause of blindness in people with diabetes and high blood pressure.

Cooking Matters: This six week cooking class series is brought to our NAHC members by 18 Reasons. Participants receive recipes and a take home bag of produce at each session. Spanish, English and Teens classes are offered annually.

VeggieRx: Brought to us by Fresh Approach, VeggieRx is a group medical visit offered to our NAHC members. This program includes eight interactive nutrition and cooking classes in which patients set a weight loss goal and learn how to enhance their health through diet and nutrition. Members that attend classes are also eligible to receive Farmers Market Vouchers. Veggie Rx is offered annually.
